Improvements

This release contains many improvements respect version 2, like:

an improved user interface for the administrator;
a simpler and customizable user interface for customers, with more filters, but that are displayed only when they make sense;
the support of incoming, outcoming and internal calls;
the support of number portability;
customers can have one or more offices and each office one or more VoIP account;
rating of calls and other administrative tasks are done off-line using configurable/customizable jobs reacting to events. This allows to customize the behavior of the application;
stats and graphs about channels usage (number of concurrent calls);
CDR table can accept Asterisk account not already configured, and they will be rated later;
